St. Joseph Nursing Home is a
non-profit, free-standing Skilled Nursing Facility. (SNF) We
accept residents with the ability to pay and those who qualify under
Federal Medicaid and Medicare guidelines.
Prior to admission, the resident
needs to have a pre-screening for nursing home placement. This
is a state requirement and can be done through ACM Care in Peoria.

The resident should also see their physician for a full
history and physical within 5 days prior to admission or 3 days
after their admission.
